H1N1 Clinic Consent Forms

If you will be bringing your child to one of our H1N1 clinics at your local school, you will need to print and fill out a consent form for each child who will be receiving a vaccination. Having the consent form filled out in advance will help the clinic to run smoothly and keep the lines short.
