1. In 2004, the Idaho birth certificate was revised. Beginning in 2004, Idaho prenatal care data are based on date of first prenatal care visit as reported in the mother's medical record. 2004 and 2005 APNCU data are not comparable with APNCU data based on month prenatal care began. The APNCU index classifies care as intensive, adequate, intermediate, inadequate, or no care by comparing the number of actual prenatal care visits that a woman was expected to receive, given the onset of prenatal care and the length of gestation. Percents based on records with stated data for APNCU calculation (date of first prenatal care visit, number of visits, and length of gestation). Data for 2004 have been corrected from the 2004 Health Indicators; 2004 data are comparable to 2005 data.

3. Prior to 2004, tobacco use during pregnancy (yes or no) was reported on the Idaho birth certificate. Beginning in 2004, cigarette smoking before and during pregnancy is reported on the revised birth certificate. Data in 2004 and 2005 are not comparable to data prior to 2004.
4. The revised 2004 Idaho birth certificate includes a new variable for reporting if mother received WIC food for herself during this pregnancy (yes or no)?

7. Motor Vehicle Crash Death Rate: number of motor vehicle crash deaths from 2003 to 2005 per 100,000 population.
